Life Insurance and Other Insurance Disclaimer

Church Pension Group ("CPG") is a trade name under which the Church Pension Fund and its affiliates serve the needs of the Episcopal Church community and does not, itself, issue insurance policies.

Life insurance

Church Life Insurance Corporation (an affiliate of CPG), NAIC No. 61875, a New York life insurance company, with its home office located at 445 Fifth Ave., New York, New York 10016 ("Church Life") offers group and individual life insurance and annuities to clergy and laypersons, and their families, in the service of the Episcopal Church.

Products and features may not be available in all states. Guarantees are based on the claims-paying ability of the issuing company. Information and descriptions of products and services are provided solely for general informational purposes and are not intended to be complete descriptions. Life insurance policies contain exclusions, limitations, and restrictions. For complete details of coverage, including exclusions, limitations and restrictions, the actual policy or certificate should be consulted. If the descriptions of the Church Life products on this website conflict with the terms of the life insurance policies or annuity contracts, then the terms of the life insurance policies or annuity contract shall govern.

Disability insurance

Short term and long term disability insurance policies are underwritten and issued to eligible clergy and lay employees by First Unum Life Insurance Company, NAIC No. 0416-64297, New York, New York as Policy Nos. 461621, 560475 and 465268 respectively. Such disability policies do not provide basic hospital, basic medical or major medical insurance.

Long term care insurance

Long term care insurance is issued by The Prudential Insurance Company of America, 751 Broad Street, Newark, NJ, 1-800-732-0416. The policy (Contract Series 83500) and Outline of Coverage/Disclosure Statement should be reviewed for terms of coverage, including benefits, limitations, exclusions, eligibility requirements and the terms under which coverage may be continued in force, or discontinued. The policy may not cover all costs associated with long term care. Coverage options and exclusions, and policy availability may vary by state. In order to be considered for coverage, you must complete and sign an enrollment form. Except for open enrollment available to eligible individuals, all other applications will be subject to medical underwriting.

The Church Pension Fund and its affiliates (collectively, "CPG") does not guarantee the performance by Prudential of its legal obligations. Premium payments under the group contract are the sole responsibility of the insureds. The information provided in this document is for educational purposes only. In the event of any conflicts between this document and the group contract documents, the group contract documents govern.
