Instructions for completing the online form:
- Enter your institution's billing information for Group Term Life Insurance in the appropriate boxes.
- Click "Add New Hire" in the gray box in the lower left corner of the form.
- Complete employee information.
- To add another employee, click "Add New Hire" again.
