Long-Term Disability Insurance 

If you receive Long-Term Disability Insurance as a benefit of employment, you are covered for 66-2/3% of your compensation, to a maximum of $5,000 per month, if you have a certified long term disability.

Benefits are reduced by any amount you receive from:

  • Workers' Compensation or similar provisions in the law.
  • State disability benefits.
  • Social Security benefits.
  • A third party by judgment, settlement or otherwise (after subtracting attorney's fees).

Because your employer pays the premium, your benefit is 100% taxable. (The monthly premium is based on your salary and is $.67 per $100 of monthly payroll.)

Once you have been insured for at least 12 months, if you leave your job you may continue your coverage by converting your policy to First UNUM Life Insurance Company and paying First UNUM at a rate they determine at the time of conversion. However, First UNUM must receive your conversion application within 30 days of your termination date.
