- Go to "DocFind"
- Select a search type and state and follow directions for Steps 1 and 2.
- For Step 3, "Select a Plan,"use the following grid to find your specific plan:

- Click on "Nationwide Provider Search."
- Click on "Physician and Hospital Search."
- Select your Health Plan: Choose "PPO"or "EPO."
- Continue with the directions on the screen.
- Select "Search the General Physician Directory"on the right side of the screen.
