It’s important to understand your options for healthcare and treatment, as well as maintaining your health. A few steps can help you take control.

Understand your healthcare benefits

Review your healthcare benefits package to understand the benefits available to you. The Episcopal Church Medical Trust (Medical Trust) offers one UnitedHealthcare® Group Medicare Advantage (GMA) (PPO) plan with two options.

Both plan options offer medical, prescription drugs, hearing, mental health benefits, among others. Dental benefits may be purchased as a separate plan.

Plan Highlights

  • UnitedHealthcare® HouseCalls (optional) – Welcome a healthcare professional to your home to review your health history and current medications, discuss health screenings, and identify health risks.
  • Telephonic Nurse Support – Speak to a registered nurse anytime about medical questions and concerns.
  • Renew Rewards – Earn rewards for completing healthcare activities.
  • Renew Active – Take advantage of this fitness program that includes free gym membership, personalized fitness plans, and online brain health activities.
  • Post-discharge transportation – 12 one-way rides up to 30 days (and no more than 50 miles each way) following any inpatient or Skilled Nursing Facility discharge. Referral required.
  • Post-discharge meals – 28 meals up to 30 days, following any inpatient or Skilled Nursing Facility discharge. Referral required.
  • OptumRx® Network – Have prescriptions filled at thousands of pharmacies across the US or delivered you via Optimum Home Delivery.
  • UnitedHealthcare Hearing – Access hundreds of name-brand/private-label hearing aids at any of 6,500 UnitedHealthcare Hearing providers nationwide. You may see any hearing provider; however, if you see a provider who is not contracted with UHC Hearing, you likely will have to pay upfront and submit a reimbursement form to UHC.
  • UnitedHealthcare Global - When traveling, the UnitedHealthcare Global Assistance Program provides medical and travel-related assistance services worldwide, 24-hours a day.

Some additional benefits offered through the Medical Trust are:

  • Cariloop Services. Quantum Health can put you in touch with Cariloop Care Coaches—doctors, nurses, social workers, occupational therapists, certified dementia practitioners, and other seasoned professionals familiar with our plans and with Medicare—who can help you and your caregivers navigate health benefits, manage care logistics, and make informed decisions about a variety of medical situations.   • The Cigna Employee Assistance Program (EAP) is a free, confidential, 24-hour service that connects you with counselors to help you with immediate health needs and with assistance for therapy referrals, tips for balancing your personal and professional life, legal consultations, financial services, and much more. It is included as part of your healthcare benefit and is also available to your covered dependents and any other household members.
  • Vision benefits offered through EyeMed's Insight Network provide coverage for an annual eye exam and cost savings on prescription glasses or contact lenses.
