Employee Paid (Voluntary) Plan

If your employer does not offer Long Term Disability Insurance, you may purchase it through your employer. Check with your employer for premium rates. Depending on your needs, you may choose coverage for either 25% or 50% of your regular compensation, up to a maximum of $5,000 per month.

If your employment ends, you may be eligible to convert your coverage as long as you have been insured for at least 12 months. Submit an Application for Conversion of Long Term Disability Insurance and your first quarterly premium to Lincoln Financial Group within 31 days of your last day of work.
